1970's New York is a lawless city plagued by drugs, violence, and murder, a "Fear City." Five untouchable mafia families hold the city in their grip, with money, control, and dominance. The law enforcement is seemingly confused about how the mob cycled money in innovative ways. The federal agents struggled to engage, especially with the family structures that were so deep the FBI could never reach to the top, to the boss.
As New York runs amok, the mob operates with impunity — until a little-known law and some innovative surveillance tactics give federal agents a leg up. The FBI had the biggest task ever, to bring down the mafia.

The mob enjoyed a golden era in the 80s. They have infiltrated every fabric of society; they had put their tentacles in legitimate areas in society including hospital, judges, ports — the mafia families were a big corporation deeply embedded in society. The FBI had decided to tap all five family squads and develop a RICO case against each of them — essentially, collecting information.
As the FBI takes daring steps to wiretap the heads of the five families, the new US Attorney Rudolph Giuliani for the Southern District of New York joins the fight.

New York was getting plenty of construction projects, worth millions and millions of dollars, that the mob was exploiting. They had a big leverage by forcing unions to go on strike which halted projects and forced people to pay them. Through the wire tappings, the FBI heard of "The Concrete Club", a billion-dollar operation for the mob families.
The federal agents knew they were on to something with the mob but they did not have a grip on the conspiracy at first, especially with The Commission. In order to prove a conspiracy among the mafia bosses, the FBI tries to tie "The Commission" to a high-profile gangland murder.
